Injury Reporting Process for IU East
Is the injured person a student, visitor or guest? Call IUPD East. Students, visitors or guests to the campus will be asked to complete the Incident Report form to report an incident.
IUPD East does an injury case report on all cases.
Is the injured person an employee? This process is applicable to all employees (Faculty and Staff).
For your injury to be considered for coverage by Worker’s Compensation, this process MUST be followed.
If it is an emergency, call 911. We’ll sort things out later.
- If it’s a non-emergency, and after business hours, call IUPD East. Notify your supervisor. Complete the Occupational Injury/Illness Report form.
- If it’s during business hours, notify your supervisor, then notify HR. If you can’t reach your supervisor,
- go ahead and notify HR.
- Whether you feel like you need it or not, we are going to have you fill out the Occupational Injury/Illness Report form.
- NOTE TO SUPERVISORS: it is ultimately YOUR responsibility to ensure that the Occupational Injury/Illness Report form has been completed.
Important Notes:
Do NOT go to your family doctor. If you do, it will NOT be covered by Worker’s Compensation. Instead, please go to the Authorized Medical Providers for IU East, listed below.
If you are working away from the Richmond area, and you are injured in the course of your duties, you can go to the nearest medical provider. You will still need to notify your supervisor, who will complete the Occupational Injury/Illness Report form on your behalf.
